Notice Exercising Option to Renew Lease

Please be advised that the undersigned, as Lessee under a certain lease for premises known as [address of the rental property] and dated [Date] (Lease), does hereby exercise his/her option to extend the term of said Lease for a period commencing on [Start date] and terminating on [End date] .
During extended term, Lessee shall pay rent of $[Amount] per annum, payable $[Amount] per month in advance, in lieu of the rent contained in the original Lease.
It is further provided, however, that all other terms of the Lease shall continue during this extended term as if set forth herein.
